EMR Parameters Setup: The drug interactions check must be enabled on the Drug Interactions tab. The sensitivity of the check should be adjusted as needed. Optionally, EMR Parameters can be configured for real time pop-up interaction alerts during history taking on the PMH tab. Medications Setup: Select all medications in the Net Health Employee Health and Occupational Medicine Medications Table from the Cerner Multum database. Confirm that medications already in the med codes table have a Drug ID, which uniquely identifies each med for the interactions check. If not, add the ID from Cerner Multum. Allergies and Conditions (PMH tab) Setup: When creating medication allergy wizards, select meds and med classes from the Multum list reached by clicking the table icon beside the related field. When setting up Active/Chronic Problems Wizards, include the ICD-9 code for items you want included in the interactions check. This might require adding ICD-9/10 codes to your Net Health Employee Health and Occupational Medicine table. Use-Allergy and Condition History: Select allergies and diseases and conditions using the properly set up wizards. Use- Medications History: For medications that are not in your Net Health Employee Health and Occupational Medicine medications table, always locate the medication by using theSelect From Multum button. Use-Medication Orders: Order meds using properly set up medication wizards, or by selecting from the medications table. If a medication is not in the table, add from Multum. Setup: Smoking/Tobacco Use Specialty Wizard must be set up and included on appropriate Templates. (It may be that the only template that needs this change is the Basic Default Charting Template.) For Net Health Employee Health and Occupational Medicine to be able to report on this item, this new Wizard type must be used. You might need to remove old smoking or tobacco use Wizards from your templates to prevent users from using them. Use: Capture tobacco use information using the Smoking/Tobacco Use Specialty Wizard on visits. For visits that are not charted, enter the smoking data manually on the Risk Factors tab of Patient Demographics. Be sure to enter the date! Setup: You must be using Surescripts to have access to eligibility and formulary information. To enable the feature, in EMR Parameters, on the Prescriptions tab, check Display Eligibility and Formulary Benefit Information during the Prescription Writing Process. Use: Eligibility and formulary benefit information is automatically displayed side-by-side with the Medication Order Wizard as a prescription is entered. Lab panel templates can be created specifically for capturing and viewing sets of labs, and medical activities can be set up to store the result for single labs. The screen for capturing and displaying lab panel results is similar in structure to the one used for capturing drug test panels. Medical Activity Setup: Create medical activities for lab test panels and/or individual lab test medical activities for as appropriate. Activity Type must be set to Lab Test for both lab panels and lab tests. For lab panels only, Trigger Entry must be set to Lab Test Panel. For lab tests only, Trigger Entry must be set to Lab Value. Wizard Setup: Add the lab panels and lab tests to appropriate Results and/or Procedures Wizards, making use of the "auto-charge" feature to automatically add the activities to the visit. It should be done in consultation with Net Health Technical Support. Setup: Modify certain system parameters as required. These include entering technical values, such as the url of the Website. They include non-technical items such as the text displayed when a patient logs in or when an invitation e-mail is sent to a patient. Use: When a patient requests access, enter a profile for that patient and send an e-mail with connection information. Use: The provider may mark lab tests, imaging/radiology activities, and diagnoses to be withheld from the patient by checking a box on the medical activity record on the visit. Net Health Employee Health and Occupational Medicine: The Export to State Immunization Registries utility does just that. Setup:You must obtain from your state the information needed to meet your state's requirements. This will typically include a provider identifier code and name that your state will need to recognize who submitted the data. There might be specific codes needed to identify the vaccine and manufacturer. There may be other requirements. If specific medical activity names or CPT codes, or other government mandated codes are required, make the needed changes in your Medical Activity and CPT Codes tables. It is essential to do this setup before you begin recording your vaccine administrations. Use: Record vaccinations using the correct codes. Use the Export to State Immunization Registries feature, found by clicking Utilities on the Home Nav Bar, to export vaccinations for a specific period. Endnotes
1) Core Items 1 & 4: CPOE and ePrescribing. Any EP who writes fewer than 100 prescriptions during the reporting period is excluded from these requirements.
2) Core Item 10:All EPs must submit calculated results for three core measures using the certified technology. You need only report the required clinical quality measures; you do not need to satisfy a minimum value for any of the numerator, denominator, or exclusions fields for clinical quality measures.
The three core measures that EPs will be required to report are:

NQF 0028: Preventative Care and Screening Measure Pair: a. Tobacco Use Assessment b. Tobacco Cessation Intervention; and

NQF0421/PQRI 128: Adult Weight Screening and Follow-up.
Insofar as the denominator for one or more of the core measures is zero, EPs will be required to report results for up to three alternate core measures:

NQF 0041/PQRI 110: Preventative Care and Screening: Influenza Immunization for PatientsGreater than or equal to50 Years Old;

NQF 0024: Weight Assessment and Counseling for Children and Adolescents; and
-
NQF 0038: Childhood Immunization Status.
If all six of the clinical quality measures have zeros for the denominators (this would imply that the EPs patient population is not addressed by these measures), then the EP is still required to report on three additional clinical measures of their choosing from a longer list of approved measures.
3) Core Item 13: Provide clinical summaries for patients for each office visit: That summary can be electronic or printed. While the must be certified to produce a CCR or CCD, meaningful use does not require that the patient actually receive their information in these formats. Paper is fine. In fact, even if you were to provide a CCR or CCD, the patient still has the option of requesting paper and you must provide paper if asked. The clinical summary can be provided through a PHR, patient portal on the Web site, secure e-mail, electronic media such as CD or USB fob, or printed copy. Regardless of the electronic media you offer, you will still be required to provide the patient a paper copy upon request.
4) Menu Items: While you must select five menu items to report, there is one limitation. All EPs must choose at least one of the population and public health measures to demonstrate as part of the menu set. This is the only limitation placed on which five objectives can be deferred from the menu set. This means that eligible professionals (EPs) must test submitting data to immunization registries (menu item 9) or syndromic surveillance registries (menu item 10). If such registries do not exist in your state/city (i.e. there is no repository to receive the data), you will not penalized by CMS.
5) Menu Item 9: Providers are excluded from this requirement if they administer no immunizations during the reporting period or there is no immunization registry has the capacity to receive the information electronically
6) Menu Item 10: Providers are excluded from this requirement if they do not collect any reportable syndromic information on their patients during the reporting period or do not submit such information to any public health agency that has the capacity to receive the information electronically.
7) Attestation: In 2011, CMS will not be prepared to accept data feeds from certified EHRs, so the primary means of demonstrating meaningful use will be via an attestation through a secure CMS website. An attestation is a legally binding representation of the accuracy of the statement. Making a false attestation could be considered fraud under the Medicare/Medicaid programs.
The provider will need to supply the results of each of the measures, as well as the certified in use. Payments will be made on a per eligible provider basis, not by practice. This means each EP will need to demonstrate the full requirements of meaningful use to qualify for the incentive payments.
8) Structured datais away of storing data in a computer so that it can be used efficiently. For our purposes, it allows the electronic exchange of information among disparate systems while assuring that all systems have the same understanding of that data. The important thing for you to know is that exchanging health care information requires converting much of it to standard codes. ICD-9/10 codes are a good and familiar example. Healthcare facilities, insurance companies and other entities that use ICD-9 codes all understand that 034.0 indicates Streptococcal Sore Throat. This eliminates the possibility of misunderstandings due to misspelling, abbreviations, and slightly different terminologies. Net Health Employee Health and Occupational Medicine employs a number of widely understood codes. CPT codes are another familiar code used for billing. UNII Codes are used to indicate types of allergies, and the VA/KP problem codes are used to indicate allergy reactions. The good news is that you do not need to know all these codes. When entering structured data in Net Health Employee Health and Occupational Medicine, you will choose from an alphabetical list of descriptions; Net Health Employee Health and Occupational Medicine will enter the correct code.
9) Withholding information from patient:Meaningful Use regulations do not seek to conflict with or override HIPAA. Therefore, an EP may withhold information from the electronic copy of a patient's health information in accordance with the regulations at 45 C.F.R. 164.524, Access of individuals to protected health information. In Net Health Employee Health and Occupational Medicine, diagnoses and results can be flagged to be withheld until release by provider. These items will be excluded from the Formal Health Record and the Clinical Summary.
