Set Up First Responder Request (Questionnaire)

Set up this medical activity to track a questionnaire used by your organization to solicit First Responders:

  1. On the Home Nav Bar, click the CODES tab. (Remember, you must have access to Codes Setup to see and access this tab.)

  2. Click the Medical Activities button.

  3. Click the Add button. On the GENERAL tab of the [Enter Medical Activity Code] window:

  4. Type an Activity Code. (For example: FRQ. The code should be as short as possible so that the code may be easily remembered and typed into the visit record, vs. having to go to the lookup list to find it.)

  5. Type the Description. (For example, "Smallpox Volunteers Request" or "Smallpox 1st Responder Q’aire. This is the description that will print on all reports).

  6. Click the down arrow to the right of the Activity Type field and select Administrative.

  7. Leave the Trigger Entry field as No Special Entry.

  8. Leave the Steps Required as 1.

If you are not going to track that the questionnaire was sent/given and returned (or not)

The Steps Required needs to be 2(Step 1 is sending/giving out the request and Step 2 is documenting the outcome).

  1. Click the STEPS tab. In the Meaning field, type "Sent" or "Given".

  2. Click the down arrow to the right of the Step 2 field, and select the time interval for when you should receive the questionnaire back. Press the TAB key.

  3. Enter in the next field the number of [Days or Weeks] for when you should receive the questionnaire back. (When you enter the result code to indicate the questionnaire was given or sent, the program will calculate a Next Due Date based on this interval.)

  4. Leave the Meaning field blank on this line. The meaning for Step 2 (i.e. the "results" of the questionnaire) will be indicated by the result code, as noted below.

  1. Click the RESULTS tab. (The results will indicate whether the employee wants to be a First Responder or not). In the Codes fields, type in:

Y and in the Meaning field type Yes.

O and in the Meaning field type No Answer (or NoRespons. No Response won't fit!)


Please note the following:

You do NOT need a code for a "No" response. The user can enter the default code R for Refused.

The O response would be used only if you are required to track the outcome of all questionnaires. If you are not going to do this, do not add the O response code, as it just complicates the medical activity.

If you are going to track that the questionnaire was sent/given and returned (or not)

Add the result code 1 and in the Meaning field type Sent or Given. (Note that you must have Steps Required set to 2 for this to work, as explained above.).

  1. Click the OKbutton to close the [Enter Medical Activity] window. If you are going to add another medical activity, repeat from step 3 above.
