To add Smallpox and Smallpox Followup to your Surveillance Profile (and the questionnaires, if desired), you first have to set them up as medical activities. You must also set up smallpox as an infections disease, and smallpox exposure as an exposure type. See the Related Items button below to access the instructions you need for this.
If you want the medical activities and medical conditions to appear on your Surveillance Profile, take these steps:

On the Home Nav Bar, click the Setup/Admin button. (Remember, you must have access to Codes Setup to see and access this tab.)

Click the Clinical Setup button.

Click the Universal Surveillance Profile button.

On the ACTIVITIES tab, click the Add button.

Search for and select the desired activity (Smallpox Vaccination or Smallpox Vaccination Followup) from the medical activity table.

Click OK.

Use the up and down arrow to position the item in the desired location on the list.

Repeat Steps 3 through 6 until you have added the medical activities to the ACTIVITIES tab.

Click the ILLNESSES tab.

Click the Add button.

Search for and select the Smallpox (illness) code from the list of medical conditions, then click OK.

If desired, use the up arrow to re-position the item on your list.

Click the Close button to close the window.
If your organization is licensed to track data for Special Populations or more than one hospital
"COMPANY" PROFILES: If your organization is licensed for Special Populations, or multiple companies (such as two or three hospital locations), AND has created different surveillance profiles for each one, you will need to alter the profile for EACH group or organization. Use the instructions above, but instead of altering the Universal Surveillance profile, click the Company/Surveillance Profile button and select the appropriate company. The rest of the instructions are the same.
If your organization has created Surveillance Profiles for any Job Classes or Locations
If your organization has created different surveillance profiles for different jobs or locations, you will need to alter the profile for EACH job/location profile that is affected. Use the instructions above, but instead of altering the Universal Surveillance profile, click the Job/Location Profile button and select (or add) the appropriate job/location. The rest of the instructions are the same.
