Reports for Smallpox Activities

If your codes are properly set up, and you have documented appropriately, the program can produce the following reports for you related to Smallpox Administration and Follow up. To get this information:	Run this report:
Who was sent a questionnaire and hasnâ€™t yet returned it?	Surveillance History Report (aka Surveillance Administration Report). Select the appropriate medical activity and click the radio button next to Otherâ€¦ and enter 1 in the text box that appears.
Who (or how many) volunteered to be a First Responder?	Surveillance History Report. Select the activity First Responder Questionnaire and click the radio button next to Otherâ€¦ and enter Y in the text box that appears.
Who is (or how many people are) qualified to be First Responders?	Surveillance History Report. Select the activity Smallpox Fitness Questionnaire and click the radio button next to Otherâ€¦ and enter F in the text box that appears.
Who (or how many people) received the Smallpox Vaccination and hasnâ€™t returned for follow-up?	If you have the Block Scheduler, run the No Shows/Cancellations report. Note that this report will be accurate only if you have marked the schedule records as No Show or Cancelled at the end of each day. If you are not using the Block Scheduler, run the Delinquent Scheduling report. Note that this report will be accurate only if, when logging a visit, you Search the Schedule and complete a scheduled visit.
Who (or how many people) received the Smallpox Vaccination and how many had adverse reactions?	Surveillance History Report. Select the activity Smallpox Vaccination and click the radio button next to Activities Administered in this period [the reporting period selected at the top].
